In fact the majority of our practice members come to our office for wellness care. Our wellness programs include preventative spinal health care, lifestyle management programs, and peak performance programs. This appointment is your initial exam and consultation to find out how our providers may help you.

Online Scheduling to assist current Patients  

Follow the one time set up process to enable online scheduling.

 Once you have enrolled as a patient in our office you can schedule your future visits through your patient portal.

You may use this portal to schedule and reschedule appointments. To cancel an appointment please call our office at 502-454-3500.

 If you are a patient of Awaken to Wellness Center and have not been seen in our office for 3 months please do not schedule through the online portal. Please call to schedule an appointment. A re-evaluation may be needed to resume care within our office. 

We request 24 hours notice to cancel an appointment or a visit fee may be charged.
