Once you have enrolled you can schedule your first visit and any future visits through your patient portal.
You may use this portal to schedule and reschedule appointments. To cancel an appointment please call our office at 502-454-3500.
All new members must call to schedule an initial appointment or use the links below to complete your intake forms and submit an appointment request.
If you are a patient of Awaken to Wellness Center and have not been seen in our office for 3 months please do not schedule through the online portal. Please call to schedule an appointment. A re-evaluation may be needed to resume care within our office.
We request 24 hours notice to cancel an appointment or a visit fee may be charged.

The doctor will review your submission and determine next steps. Once the doctor has reviewed your forms, we will follow up with you during our next business day to schedule your New Member appointment or make recommendations to you by phone. This critical step allows us to make sure our office believes we are the right fit for your health and wellness needs. Ultimately, this step will save you time and money on your health journey. What to Expect?
Expect Compassion
Our goal is to find the cause behind health problems that are keeping you from experiencing life at its fullest.
What to expect during your first visit:
- Expect to be treated as the amazing person you are.
- Bring with you a driver’s license and health insurance card.
- A one-on-one consultation with one of our chiropractors will be included; a neurological and spinal assessment, along with range of motion testing and a discussion of your health issues and goals will be performed.
- When necessary, we will take specific chiropractic postural x-rays to evaluate the spine. If you have previous x-rays or imaging from other providers please bring them to your appointment, as we support a conservative approach to taking new ones.
- Nervous system scans will be done measuring nerve to muscle communication by utilizing Surface EMG technology and nerve to organ communication by utilizing Thermal Technology.
- When appropriate functional lab testing may be ordered to further evaluate any nutritional needs that may help you heal well.
What to expect on your second visit:
- Your provider will go over your examination findings with you, which is referred to as a Report of Findings. This will include reviewing your health and healing with you in more detail. We will give you a full diagnosis and discuss a recommended care plan for you. If we do not believe we can help you we will help you find the providers that can.
- If we recommend care and you choose to begin care, you are welcome to be adjusted or receive the recommended services that day and begin your road to regaining or improving your health!
